You can now request an Education, Health and Care Needs Assessment online using the portal.
You can also gain access to a dashboard to:
- Track your progress through the Needs Assessment process
- See when your next Annual Review is due if you have an EHC Plan
- See who your allocated Family Services worker is, and send them a message
- View documents sent to you, such as letters
- You can use the portal on your smartphone or tablet

“How do I use the portal and dashboard?”
- You can begin an EHC Needs Assessment request on the portal at any time. You don’t need to complete the request in one go; once started, your form can be saved on your portal account for 30 days.
- If you would like support using the portal to request an EHC Needs Assessment please contact the EHCNA Request Team on 01473 263688 or EHCNARequest@suffolk.gov.uk.
- If you already have an Education, Health and Care Plan and would like to gain access to the EHC dashboard, please contact your allocated Family Services Worker.
