What is an Education, Health and Care (EHC) plan?
It’s a document that outlines a child or young person’s special educational needs and disabilities (SEND) and the support they need. It describes a child’s educational, health, and social care needs, as well as their desired outcomes and aspirations.
An EHC plan is created when a child or young person undergoes an assessment with their educational setting.

Young people can now request an Education, Health and Care Needs Assessment online using the portal.
You can also gain access to a dashboard to:
- Track your progress through the Needs Assessment process
- See when your next Annual Review is due if you have an EHC Plan
- See who your allocated Family Services worker is, and send them a message
- View documents sent to you, such as letters
- You can use the portal on your smartphone or tablet

“How do I use the portal and dashboard?”
- You can begin an EHC Needs Assessment request on the portal at any time. You don’t need to complete the request in one go; once started, your form can be saved on your portal account for 30 days.
