This is where your Social Worker (if you are aged 16-18) and Personal Advisers (if you are aged 18-25) will work from.

They will support you with your Pathway plan – all of your worries, wishes and dreams should be in the Pathway plan.

What support and I entitled to?

As a care leaver, you’re entitled to receive different types of support. Visit the Children’s Commissioner websiteto find out more about the support that you might be entitled to as a care leaver.  

When you are 16 you will gain a leaving care worker who will help you think about your next steps. This might be going to college or university.

Access to records

You don’t have to pay and you can receive support for your emotional wellbeing in the process. It’s a BIG DEAL!

Through our experience of working with care leavers we recognise that accessing your records is often a really difficult process to go through – make sure you have support around you” .
